What is the Changemaker Award?

Each year, the DuPage County Health Department and the Prevention Leadership Team honor individuals, organizations and communities who are leading the way in prevention efforts and going above and beyond to protect adults and youth in DuPage County by making a positive change in the community. 


Who is Eligible?

See below for criteria that would make someone eligible to receive a Changemaker Award.

Teen Changemaker

  • DuPage County resident, 18 years or younger.
  • Actively involved in efforts that promote mental wellness or prevent substance misuse among their peers.
  • Demonstrates leadership and inspires other youth to take part in prevention-focused initiatives.

Adult Changemaker

  • DuPage County resident, 19 years or older.
  • Contributes to prevention, education, or advocacy efforts that support mental wellness and/or reduce substance misuse for youth and/or adults.
  • Serves as a role model by collaborating with others and going above and beyond to create a healthier community.

Organization Changemaker

  • Organization located in or serving DuPage County.
  • Dedicated to advancing mental wellness and/or preventing substance misuse through programs, policies, or community partnerships.
  • Demonstrates innovation and leadership that sets an example for other organizations in promoting a prevention, treatment, or recovery focused culture.

Community Changemaker

  • City, town, village, or neighborhood within DuPage County.
  • Implements and sustains policies, practices, or campaigns that promote mental wellness and/or prevent substance misuse for residents.
  • Leads the way for other communities by modeling collaborative prevention, treatment, or recovery centered approaches.


How to Submit a Nomination

Nominations must be submitted through this form by Monday, February 2, 2026.


When are the Winners Announced?

The Changemaker Awards will be presented in person during the GPS Parent Series event on Tuesday, February 24th at the Glenbard West High School Library.
